Listed below are the forms you can choose to purchase. Each of our "offline" forms are a one-time cost of $15 EACH and will be manually emailed to you within one business day once you have purchased them.

Note: Upon receipt of your order, we will will email you your chosen forms. Once you receive your forms, please read the tips below to get started. Your forms can be saved on your computer in PDF format or you may upload the PDFs you create and save on your computer to the existing Electronic Health Record (EHR) system you use (please note that these systems have their own instructions for how to upload PDFs into them).

Once you have purchased and received your selected STEPnotes forms, you will find the following "getting started tips" helpful:

✔ You will need to be sure to have the latest version of Adobe Acrobat Reader downloaded and installed on your computer to use in conjunction with the STEPnotes PDF forms.
✔ Always be sure to open the STEPnotes PDF forms with Adobe Acrobat Reader vs. your Internet browser's Adobe plug-in or another third party PDF utility so that you can take advantage of all available features.
✔ As the user and owner of your documentation, you are responsible for abiding by any relevant HIPAA laws and regulations. Always be sure to save a set of the original PDF files in a HIPAA compliant location on a secure computer (or you may upload the completed and saved PDFs of your notes to the Electronic Health Record (EHR) system you use), in case you need to refer back to them (should you accidentally overwrite one or more of the STEPnotes forms.)
✔ Always be sure to save new copies of the STEPnotes PDF forms as you complete them for your clients (named according to your liking).
✔ The STEPnotes PDF forms are provided to you for use at your own discretion. Please review and understand the overall STEPnotes Terms of Use agreement.
✔ It is important for those who purchase the forms to note that STEPnotes™ is trademarked and protected by law; therefore, the STEPnotes™ format may not be duplicated or copied in any manner, and the forms that are sold can only be used by the person who purchased them. The PDF forms are not to be distributed, shared, replicated or copied in any manner and for the sole use of the person who paid for them. Those who violate these terms are in violated of copyright and trademark agreements and may be subject to legal action.
